2025 First Holy Communion

Thank you for filling out the 2024-2025 Sacramental Form for your child receiving the Sacraments of First Reconciliation and First Holy Eucharist. This form ensures we have the correct student and parent information. For children not baptized at St. Raymond Parish, we ask that you upload a copy of your child’s baptism certificate to complete the form. Please be prepared with a JPEG, PNG, or PDF copy of your child’s certificate when filling out this form. Please complete it no later than Friday, November 15, 2024.
