2026 First Holy Communion

Thank you for filling out the 2026 Sacramental Form for your child receiving the Sacraments of First Reconciliation and First Holy Eucharist. This form ensures we have the correct student and parent information. For children not baptized at St. Raymond Parish, we kindly request that you upload a copy of your child’s baptismal certificate to complete the form. Please be prepared to provide a JPEG, PNG, or PDF copy of your child’s certificate when completing this form. Please complete it by Friday, October 31, 2025. The sacrament form must be completed to receive the registration link for First Reconciliation Services (released on 11/3/25) and First Holy Communion Masses (released on 12/12/25)

As a parent or guardian, I authorize the treatment by a qualified and licensed medical doctor of the minor(s) listed in this registration in the event of a medical emergency that, in the opinion of the attending physician, may endanger his/her life, cause disfigurement or undue discomfort if delayed.
