Ages: Pre-K4 through 5th-grade

Cost: $50 per camper

The on-campus program offers safe and faith-filled activities to captivate the youth through 5 stations, including games, bible stories, music, crafts, and snacks. In case of serious injury or illness, I authorize St. Raymond de Penafort staff to take emergency action deemed necessary, including the transportation of my child to a hospital or medical center. I authorize the treatment by a qualified and licensed medical doctor of the minor(s) listed in this registration in the event of a medical emergency that, in the opinion of the attending physician, may endanger his/her life, cause disfigurement or undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to reach me. This release form is completed and signed on my own free will with the sole purpose of authorizing medical treatment under emergency circumstances in my absence.
